Each sample pack will come with the listed samples wrapped in ready-to-gift bags with my business card and a custom discount code to offer to your patients/ clients for use on my website.

Here's how it works (PLEASE READ FIRST):

1. Complete this form to submit your provider information. On the response page, you will automatically get a 50% off code to use at checkout, so don't click out too fast!
2. Select your desired sample packs from the dropdown menu.
3. Your sample packs come with my business card and a custom discount code to offer to your clients/ patients to use in my shop. I will use a default code, however, if you would like to have a specific discount code to provide for your clients/ patients, please put that in the Notes at checkout.
